From the 28th to the 30th day, a control bone marrow puncture is planned. We expect positive results. Now Alena is not feeling her best - the main blood counts are below normal, her left arm hurts very much, it is swollen and weak, it works, painkillers help only for a short time. It is especially difficult at night - for some reason the pain worsens at night. Every day we talk to the doctor about this and he selects different painkillers. For example, morphine is used very carefully so as not to cause side effects and addiction. Unfortunately, Alena has experience of weaning off morphine - this is a very difficult and long process. The doctor explains such severe pain by the breakdown of tumor cells and this period must be survived - which is what Alena is doing. At the beginning of the week, the doctor introduced several targeted drugs into the therapy process. The dose is minimal for now - they are looking at the body's response and the manifestation of side effects. Also, I do not have information about the start date of the 2nd course of car-t therapy with CD 19 cells. I think that the doctor will announce this information after the puncture next week.
Despite the fact that she cannot leave the ward, Alena spends her day actively - she paints pictures, assembles a construction set, edits clips, records videos, prepares for the Unified State Exam - after all, in September Alena will go to study in the 11th grade.
Unfortunately, there is also difficult news for us - the doctor confirmed the need for a repeat BMT. This time, the repeat BMT must be from an unrelated donor with a 10/10 suitability. It is a very difficult task - to find a suitable donor, activate, and the most difficult thing is not to waste time (BMT should be performed within 2-3 months after the end of the course of car-t therapy), and to approach the transplant as prepared as possible (complete remission, normal body weight, minimized gastrointestinal problems). In China, they have already started searching in the Chinese donor database and we have been billed for BMT. It is 180 thousand dollars. This is an unaffordable amount for us.

Z jednej strony tak szybko, a z drugiej długo i trudno, przebiegała terapia Car-T. W przyszłym tygodniu wchodzimy już w fazę końcową – we wtorek, 28 dni po wstrzyknięciu komórek. Od 28. do 30. dnia planowane jest kontrolne pobranie szpiku kostnego. Oczekujemy pozytywnych rezultatów. Alena nie czuje się teraz najlepiej – morfologia krwi jest poniżej normy, bardzo boli ją lewa ręka, jest opuchnięta i osłabiona, ale działa, środki przeciwbólowe pomagają tylko na krótko. Jest szczególnie ciężko w nocy – z jakiegoś powodu ból nasila się w nocy. Codziennie rozmawiamy o tym z lekarzem, który dobiera różne środki przeciwbólowe. Na przykład morfinę stosuje się bardzo ostrożnie, aby nie powodować skutków ubocznych i uzależnienia. Niestety, Alena ma doświadczenie w odstawianiu morfiny – to bardzo trudny i długotrwały proces. Lekarz tłumaczy tak silny ból rozpadem komórek nowotworowych i konieczność przetrwania tego okresu – co właśnie robi Alena. Na początku tygodnia lekarz wprowadził do terapii kilka leków celowanych. Dawka jest na razie minimalna – obserwują reakcję organizmu i występowanie działań niepożądanych. Nie mam również informacji o dacie rozpoczęcia drugiej kuracji Car-T z komórkami CD 19. Myślę, że lekarz ogłosi tę informację po punkcji w przyszłym tygodniu.
Pomimo że nie może opuszczać oddziału, Alena aktywnie spędza dzień – maluje obrazy, składa zestaw konstrukcyjny, montuje klipy, nagrywa filmy, przygotowuje się do egzaminu państwowego – w końcu we wrześniu Alena pójdzie do 11 klasy.
Niestety, mamy dla nas również trudną wiadomość – lekarz potwierdził konieczność ponownego przeszczepu szpiku kostnego. Tym razem przeszczep musi pochodzić od dawcy niespokrewnionego, z 10/10 zdatnością. To bardzo trudne zadanie – znaleźć odpowiedniego dawcę, aktywować go, a najtrudniejsze to nie tracić czasu (przeszczep szpiku kostnego powinien zostać wykonany w ciągu 2-3 miesięcy od zakończenia terapii kardio-T) i podejść do przeszczepu jak najlepiej przygotowanym (całkowita remisja, prawidłowa masa ciała, zminimalizowane problemy żołądkowo-jelitowe). W Chinach rozpoczęto już poszukiwania w chińskiej bazie dawców i wystawiono nam rachunek za przeszczep szpiku kostnego. Wynosi on 180 tysięcy dolarów. To dla nas nieosiągalna kwota.
